In order to request vacation from a Pediatrics rotation, please follow the procedure outlined below:

1. Requests must be sent by email to This e-mail address is being protected from spam bots, you need JavaScript enabled to view it
2. Requests must have the word VACATION as well as your last name within the subject.
3. Each individual vacation request MUST be made by separate email.
4. Within the body of the email include:
   • Who you are
   • What service you are from
   • What rotation you will be on during the requested times
   • The requested dates
   • Whether it is vacation or conference leave
5. Vacation requests not complying with the above rules will not be considered official and will result in a vacation request being missed, delayed or denied.
